However, the level of benefits you receive is within your control, based on which providers you choose and whether they fall into the Select or Independent Option. |

If a claim is denied in whole or in part or if you disagree with the benefit payment on a claim, you or your Provider may request a review of the decision. |

Hospital stays, day surgeries, physical therapy and other major services and supplies must be reviewed in advance to help ensure that your treatment is appropriate and reasonable. Medical and Behavioral Health professionals at organizations under contract to Boyd Benefits conduct these reviews. This service is called Preauthorization. |
